   - Best parent/guardian contact number
   - Parent/guardian post secondary education level

• Personal Information
   - Email
   - Cell phone
   - Address
   - Current middle school or high school
   - Write a short essay explaining why you want to attend Early College

• Teacher information
   - Names and email addresses of six teachers (Remember, spelling is very important. The application will automatically email your teachers.  Misspelled email addresses will result in delays processing your application.)
